OH Medical is currently looking for two Remote Case Managers to join our market-leading Rehabilitation client on a full-time, permanent basis.
This will be a homebased vacancy, however, you will be required to travel to sites as and when required. A candidate located in the South or the Midlands with good commutable links would be ideal.
Key Tasks and Responsibilities:
Manage the case management for individuals who have sustained complex orthopaedic injuries or a traumatic / planned amputation.
Assess the individual’s needs to develop and manage a treatment plan and overall return to function and work.
Work closely with the individual, health professionals and their employer to achieve positive outcomes.
Write findings and recommendations in reports to stakeholders.
Management of a financial delegation per case.
The role is located in the South of England as a home-based worker. However, there will be a requirement for travel when needed.
Criteria:
HCPC or NMC registered (Essential)
Driving licence & your own vehicle (Essential)
Rehab Case Management experience (Essential)
Minimum of 2 years clinical experience (Essential)
Salary & Benefits:
£32,000 - £35,000 per year.
Car Allowance (£4,000 per year).
